Register With The Practice?

New Patients

If you would like to register with the practice, we can only accept you if you live in the practice area, as defined on this website. You would need to attend the surgery with proof of address and you would be given a health screening appointment with the registration clerk. In addition you would also be given a registration form which must be filled in and you would be required to bring a urine sample when you come for the appointment. You would need to tell us details of your past medical history/operations/accidents in addition to a few other personal details. In the case of women, we would be grateful if you would inform us if you have had a cervical smear in the last five years and the result of this. Please be aware, if you are not able to attend a health screening appointment please cancel and rebook as failure to do this would result in non-registration.
